How will I pay for treatment? It's one of the first questions people ask before booking physiotherapy or chiropractic care. The good news is that many people in Ontario have at least some coverage through workplace or private health benefits. Here's a plain-language guide to your options.
Extended health benefits
Most people pay for physiotherapy and chiropractic care through an extended health benefits plan from their employer, a spouse's employer or a private policy. Plans usually set an annual dollar maximum for each type of practitioner, and some also limit the amount paid per visit or the number of visits per year.
Before your first appointment, find out:
- How much coverage you have for physiotherapy and for chiropractic care (these are often separate amounts)
- Whether there is a per-visit maximum, or a percentage you pay yourself
- Whether your plan needs a doctor's referral to reimburse treatment
- When your plan year resets
You'll find these details in your benefits booklet, on your insurer's app or website, or by calling your insurer.
What is direct billing?
With direct billing, the clinic submits the claim to your insurance company for you at the time of your visit. Your insurer pays its share directly to the clinic, and you pay only the difference, if any. It saves you from paying the full amount up front and submitting receipts yourself.

Does OHIP cover physiotherapy or chiropractic care?
OHIP covers physiotherapy only in limited circumstances and only at designated, publicly funded clinics. For example, some people aged 65 and older or 19 and younger, and some patients who have just been discharged from hospital, may qualify. These programs usually require a referral from a doctor or nurse practitioner, and the Government of Ontario's website (ontario.ca) lists the current eligibility rules.
Chiropractic care is not covered by OHIP. Most people pay for chiropractic visits through extended health benefits or out of pocket.
Health spending accounts and tax receipts
Some employers also offer a health spending account (HSA), which can cover costs your regular plan doesn't. Keep your receipts, too: physiotherapy and chiropractic fees you pay yourself may qualify for the federal medical expense tax credit. Check the Canada Revenue Agency's guidance or ask your tax preparer.
Paying out of pocket
No benefits? You can still book. Ask about fees when you book so there are no surprises. A clear plan with home exercises also helps you make the most of every visit, so you only come in as often as you need to.
