Do You Need a Doctor's Referral for Physiotherapy or Chiropractic Care in Ontario?

It's one of the questions we hear most often: do I need a doctor's referral before I can book? For most people in Ontario, the answer is no. Physiotherapists and chiropractors are regulated health professionals who can assess and treat you directly, so you can book your first appointment yourself.

There are a few situations where a referral or doctor's note still matters, but they are usually about how your care is paid for rather than whether you're allowed to be seen. Here's what to know before you book.

Physiotherapists and chiropractors are direct-access providers

In Ontario, physiotherapists are regulated by the College of Physiotherapists of Ontario and chiropractors by the College of Chiropractors of Ontario. Both professions are trained to take a detailed history, examine you, work out what is driving your symptoms and build a treatment plan. Neither needs a physician's sign-off before seeing you.

When a referral or doctor's note can still matter

  • Extended health benefits: Some insurance plans ask for a physician's referral before they will reimburse physiotherapy or chiropractic visits. Rules vary from plan to plan, so check your benefits booklet or call your insurer before your first visit.
  • OHIP-funded physiotherapy: OHIP covers physiotherapy only in limited circumstances and only at publicly funded clinics, and those programs usually require a referral from a doctor or nurse practitioner.
  • Workplace injuries and car accidents: Claims through WSIB or auto insurance have their own processes and paperwork, so let your insurer and your clinician know about a claim early.
  • Medication, tests or specialist care: If your physiotherapist or chiropractor thinks you need medication, further tests or a specialist's opinion, they will recommend that you see your family doctor.

When to see a doctor first

Most aches, strains and sports injuries are a good fit for physiotherapy or chiropractic care, but some symptoms need medical attention first. Get medical help right away (call 911 or go to the nearest emergency department if symptoms are severe or come on suddenly) if you have:

  • New loss of bladder or bowel control, or numbness around the groin or inner thighs
  • Rapidly worsening weakness or numbness in your arms or legs
  • Back or neck pain after a serious fall, car accident or other significant trauma
  • Back pain together with fever, unexplained weight loss or a history of cancer
  • Chest pain, shortness of breath, or a sudden severe headache unlike any you have had before

If none of these apply, it is usually fine to start with a physiotherapist or chiropractor. If anything during your assessment suggests you need a doctor, we'll tell you and help you arrange the next step.
